Procedure:

- For letters, please email the office with details of your request: mdelaney@medicorcancer.com
- Please confirm we have all the information needed to complete the form or write your letter
- Purchase this item (quantity = 1 for each simple form / letter)

- Complete the order
- Our office will receive your request via email, normally within minutes
