Completion of a form, for example, report requested by employer or insurance company. Intermediate complexity form, 1 – 2 pages. Also applies to intermediate complexity 1 – 2 page letters written by our nurse practitioner. HST taxable (not a medical service).

- For letters, please email the office with details of your request: mdelaney@medicorcancer.com
- Please confirm we have all the information needed to complete the form or write your letter
- Purchase this item (quantity = 1 for each intermediate form / letter)
