This limited consultation is for patients who wish to transfer an existing care plan to the Medicor / Sano Via office. It includes a review of medical history, targeted examination and review of relevant medical reports. It includes confirmation of safety and appropriateness an existing care plan, which can then be continued at Medicor / Sano Via Wellness.
Please download and complete our New Patient Forms and return them to the office by email or fax. After we receive the forms and payment is confirmed, an appointment will be arranged.
